Every immune system needs two things: a way to attack threats, and a way to know when to stop. That second part — the “stand down” signal — is the job of a specific type of cell called a regulatory T-cell, or Treg. Tregs are directed by a single gene: FOXP3.

When FOXP3 signaling is strong, your immune system can tell the difference between a real threat and your own tissue. When that signaling breaks down, the line blurs — and the immune system can begin attacking joints, thyroid tissue, skin, the gut lining, or other parts of the body, depending on the condition.
